How to Register

To register with our practice, please come into reception to get a form, or follow the link below to download the GMS1 New Patient Registration form, which you can print off and bring into the surgery;

You must attend the surgery with 2 forms of identification (ID) when handing in your Registration Form.

Acceptable Identification: tenancy agreement, mortgage agreement, bank statement, driving licence, passport, birth certificate etc.
